Technical Assistance:
Discussed Standard 560.G - dating/labeling of food from home.

Comments:
An unannounced monitoring inspection was conducted on 11/15/2018 from 9:50am - 12:45pm. There were 86 children in care with 15 staff. The children ranged in age from infant - six years. Children were observed during morning program time, lunch and nap. Records were reviewed for nine children and five new staff. Two medication reviews conducted. The violations are listed on the violation notice and were reviewed with the director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review and interview, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The most recent immunizations on file for child 2 were administered on 10/05/2016. a. Administrative staff verified that updated immunizations had not been updated for child 1.

Plan of Correction: All infant, toddler and two year old shot records will be audited on or before January 1, 2019 by school management with a tickler form set up to notify in advance of expirations.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center failed to ensure that children's records contain the address for two emergency contacts. Evidence: 1. The enrollment record for child 1 lacked an address for the two designated emergency contacts. a. Administrative staff verified that the parent had not provided an address for each contact.

Plan of Correction: No child will be accepted into care without a full paperwork appointment. Management is to oversee this process with DM doing spot checks quarterly effective December 1, 2018.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas of the center shall be maintained in a safe condition. Evidence: 1. Areas of peeling paint was observed on the area of the alliance wall surrounding the toilets in the two-year old bathroom and early preschool bathroom. a. This area of wall is low and within reach of a child using the toilet. b. Peeling paint is a safety hazard if picked at and ingested. Additionally, the peeling paint can prohibit the ability of staff to properly clean the wall area surrounding the toilets.

Plan of Correction: We are working in conjunction with our facilities department to correct these items on or before Dec. 30, 2018. Our safety captain will be retrained to note these deficiencies during weekly audits.

Standard #: 22VAC40-185-560-G
Description: Based on observation, the center failed to ensure that when food is brought from home the food container shall be labeled in a way that identifies the owner. Evidence: One box of infant cereal was not labeled with the name of the the child the cereal belonged to. a. The box was placed in a name labeled area of the cabinet however, the box did not contain any name label.

Plan of Correction: All infant teachers will be retrained on ensuring labeled items only come into the school, on or before December 1, 2018. School management will check weekly for compliance.
