Comments:
The licensing inspectors conducted an unannounced monitoring inspection on Monday, October 29, 2018 from approximately 9:56 am to 3:45 pm to determine the center's compliance with licensing standards. The children in care were observed by the licensing inspector in the various classroom settings. The census for today's inspection consisted of 174 children and 25 staff. There were additional staff on site for administrative and support purposes. The program's theme for today was jersey day. The licensing inspectors observed the children in care completing a variety of activities to include engaging in active play with peers and age appropriate equipment, hand painting, working on the letter G, identifying orange things and painting each item, eating snack, singing songs in circle time, actively participation in music class, participating in the book fair, returning from a field trip, learning about money, completing bathroom breaks, and transitioning for lunch time. The children's lunch for today consisted of chicken pot pie, oranges, and milk. The center's classrooms, bathrooms, and outdoor playground areas were inspected. During the inspection, the following information was reviewed by the licensing inspector: daily attendance, emergency supplies, injury reports, fire and shelter-in-place drills, liability insurance policy. There were six children?s records and six staff's record reviewed during this inspection. The required posting were inspected. Medication is being administered, medications, and written authorizations were reviewed for the center's compliance. Standard #: 22VAC40-185-160-A
Description: Based on review of staff records and interview with staff, the center did not ensure each staff member submitted documentation of a screening 21 days after employment/volunteering or within 12 months prior to.

Evidence: During the inspection conducted on October 29, 2018, in Staff#'s 5 (DOH: 06/15/15) the documentation of a tuberculosis screening was not found. Interview with staff revealed the staff has not completed screening at this time.

Plan of Correction: The center will ensure staff will complete TB screening on October 30, 2018

Standard #: 22VAC40-185-270-A
Description: Based on observations, the center did not ensure all areas and equipment of the center were maintained in a clean, safe, and operable condition.

Evidence:
1. During the inspection conducted on October 29, 2018, the following observations were completed at the outdoor playground areas:
a. Three plastic buckets and a toy truck were observed with multiple cracks located in the wooden storage area on the Preschool and Pre-K playground.
b. Three wooden log barrier surrounding the playground areas were observed with deteriorating wood in which two logs exposed protruding nails.

c. There were three climbers with visible rusting in multiple areas on the Preschool and Pre-K playground area.
d. The swing set chains on upper and lower levels were rusting.

Plan of Correction: The center will ensure maintenance address all of physical plat by November 6, 2018.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff record and interview with staff, in one of six records, the center did not ensure to obtain a central registry finding within 30 days of employment or volunteer service.

Evidence: During the inspection conducted on October 29, 2018, in Staff#5's (DOH: 06/15/15) record, there was no central registry finding found in the record. Interview with staff confirmed the staff central registry finding has not been completed at this time.

Plan of Correction: The staff has competed the CPS form today and it will be mailed today.
