An unannounced monitoring inspection was completed on 11/26/2018. Present during the inspection were 22 children, ages 19 months-10 years, in the care of 7 staff members. The inspector reviewed 5 children's records and 5 staff records during today's inspection. The program director stated that medication is not being administered at this time. The children were observed in free choice activities, circle time, during lunch and during nap. The inspector reviewed the emergency preparedness plans and emergency supplies during the inspection today. The licensing inspector discussed the following with the staff: emergency plans, the license, medication, the physical plant, subsidy requirements and background check requirements. Description: Based on observations, the facility failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Evidence: 1. There is a foosball table located in the gym area of the center; the foosball table has sixteen handles (8 on each side) used to play the game. Fifteen of the sixteen handles on the foosball table are missing the rubber covers and expose a sharp edge with a hole in the center. 2. There is a broken cubbie /center system in the gym area of the center; the cubbie/center system has a broken shelf that has exposed 4 protruding nails that are accessible to the children. 3. Children were observed playing in the gym area of the center during the 11/26/2018 inspection.

Plan of Correction: The shelf will be repaired and the foosball table will be removed.

Standard #: 22VAC40-185-290-3
Description: Based on observations, the facility failed to ensure that electrical outlets shall have protective covers when serving children of preschool age or younger. Evidence: There was an uncovered electrical outlet located beside of the water fountain in the entrance hallway and an uncovered electrical located by the large flat screen television located in the gym. Children were observed in both areas of the uncovered electrical outlets during the 11/26/2018 inspection.

Plan of Correction: The Director will do a walk through each day.

Description: Based on observations and record review, the facility failed to ensure that no person shall be hired for compensated employment prior to the facility obtaining an employment eligibility letter upon the completion of a fingerprint background check as required by the Code of Virginia. Evidence: The personnel record for Staff #4 has a documented hire date of 11/05/2018; the fingerprint eligibility letter was dated 11/07/2018. Staff #4 was observed working on 11/26/2018.

Plan of Correction: Upon application and desire of hire potential employee will be sent for fingerprinting, upon receipt of email hiring process will begin.
