Reviewed requirements for wording on children's medication authorizations.

An unannounced monitoring inspection was conducted today. There were 72 children in care, with a total of 16 staff members assigned to their direct supervision. Children's records, new staff records, staff training, children's medications, the physical plant, classroom programming, emergency preparedness items, and administrative documents were inspected today. The children were observed attending music class, participating in The Very Hungry Caterpillar projects, listening to stories, playing outside, and eating snack. A fire drill was also conducted while the Inspector was on site today. This inspection was completed between approximately 10:15am and 12:00pm. Thank you for meeting with me today. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, it was determined that one record is not complete. Evidence: Child record C does not include the information to identify the father's place of employment.

Plan of Correction: I will ask the parent to update the information in the file.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that one potentially hazardous substance was not stored in a locked manner. Evidence: In the art room, a container of liquid starch labeled "keep out of the reach of children" was stored inside a cabinet that was not locked.

Plan of Correction: The item was immediately locked during the inspection. All staff will be reminded to keep these items locked.

Standard #: 22VAC40-185-510-C
Description: Based on a review of children's medications, it was determined that one long term authorization form does not include the physician's signature. Evidence: Child A has one non-prescription long term medication on site. The authorization form for this medication does not include the physician's signature.

Plan of Correction: The parent will be asked to have the form completed correctly.
