The licensing inspector conducted an unannounced renewal inspection 11/15/2018 from 10:43am to 2:15pm. The children were observed participating in watching a movie, reading books, reciting nursery rhymes, eating lunch, and napping. Lunch consisted of hamburgers on a bun, kale greens, mixed fruit and milk. The lunch menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today. The facility was equipped with manipulatives and supplies, and items were available to the children. Medication is not administered at this time. However, a staff person does have E-MAT training.

Five children?s records and five employee records were reviewed during this inspection. Ten injury reports were reviewed.

The center's first aid kit and emergency supplies were inspected.

The violations from the previous inspections were checked for corrections. There was one repeat violation found during today?s inspection.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

The Department has not yet received a complete renewal application and annual fee from the provider. A complete renewal application and fee must be received by the Department prior to expiration of the license on January 05, 2019 or the license cannot be renewed.

Standard #: 22VAC40-185-160-C
Description: Based on observation, the center did not ensure that staff resubmit documentation of TB test results every two years from the date of the first initial screening or testing. Evidence: The record of staff #3 (start date: 6/1/2011) had documentation of TB results dated 4/30/2016. Staff must resubmit TB results every two years.

Plan of Correction: Instruct staff member that a TB test is needed in their file by the end of the month.

Standard #: 22VAC40-185-340-F
Description: Based on observation, the center did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: The inspector observed staff #3 periodically leave the three year old children alone or with a volunteer for three to five minutes at a time to assist children in the restroom.

Plan of Correction: Bathroom routine is being reevaluated. Staff members will be given specific instructions on not leaving the children alone with a volunteer. A bathroom routine will be established where students are monitored in the classroom and bathroom the whole time.

Standard #: 22VAC40-185-550-M
Description: Based on a review of documentation on 11/15/2018, the center did not ensure that the record of injuries contained all of the required information. EVIDENCE: 1. Out of a sample of 10, six injury reports did not contain the time the parent was notified. 2. Out of a sample of 10, five injury reports did not contain the date the parent was notified.

Plan of Correction: Injury reports were edited at the time of Licensing visit. Parents are always notified during pick up, unless in case of an emergency or extenuating circumstances.
