As discussed, until receipt of all background checks on the board officer, the issuance of your license will remain pending. Discussed allergy action plans and how they differ from a medication authorization.
An unannounced Renewal inspection was conducted this date. A total of 72 children with 15 staff on site today. Observation of all 7 classrooms was made. Staff interviews conducted, observation of programming, hand washing and diapering procedures were observed. The children were observed transitioning to outside play, painting at table tops and decorating the artwork with sprinkles. Infants were observed being held while fed, rotated in their positions and provided tummy time. Lunch was also observed. Lunch consisted of chicken nuggets, corn, honeydew and milk. Some medications were reviewed. In the Pre-K class children were in a circle and reviewed their journals to the group. The letter "E" is the letter of the week. Previous violation were reviewed from the May inspection, corrective actions were verified. This inspection began at 10:30am and ended at 2:50pm. Thank you for your cooperation today. Standard #: 22VAC40-185-130-B Description: Based on review of child records, updated immunizations were not obtained. Evidence: Child#4 is under the age of 2 years. The last documented immunizations are dated 7/2017. Documentation of updated immunizations are required every six months for children under the age of 2 years. Plan of Correction: We will request this documentation from the parent of this child.
Standard #: 22VAC40-185-60-A Description: Based on review of children's records, one out of 7 records reviewed did not have all required documentation. Evidence: Child #4's record did not have documentation of a completed health exam. Plan of Correction: We will request a new physical for this child.
Standard #: 22VAC40-185-270-A Description: Based on observation of the center, areas of the building was not maintained in a safe and operable condition. Evidence: 1. Tricycles located off the Infant 2 classroom have handle bars that are rusted and exposed. 2. The gate located off the Infant 2 playground has paint that is peeling. This gate is within reach of children in care. 3. The lower left corner of the exterior door in the Toddler class has rust and peeling paint. 4. The chalkboard located in the Intermediate class is falling off the wall. This chalkboard is within reach of the children in care. Plan of Correction: Work order requests have been submitted for the areas of the building needing repair. The chalkboard will be removed.
Standard #: 22VAC40-185-330-B Description: Based on observation of the outside playground area, the resilient surfacing does not comply with the minimum safety standards. Evidence: The fall zones located on the playground does not maintain a minimum of 6 inches of mulch as required by the minimum safety standards. Plan of Correction: Mulch was ordered and expected delivery is tomorrow.
Standard #: 22VAC40-185-350-F Description: Based on observation of the Intermediate Class and staff interviews, a child was assigned to a different age group without the required documentation. Evidence: Child #7, age 2, is transitioning to the 3 year old classroom. No documentation was found of the parent's written permission or a written assessment by the program director and program leader. Plan of Correction: This child turns 3 years old in 2 weeks. Until then, we will obtain the documentation.
Standard #: 22VAC40-185-500-B Description: Based on observation of the Infant 2 classroom, the diapering surface was no longer non-absorbent. Evidence: The diaper mat in this classroom had a crack on the side of the mat and is no longer non-absorbent. Plan of Correction: The surface was replaced during the inspection with a new one.
Standard #: 22VAC40-185-560-F-4 Description: Based on observation of the Infant 2 class, food was served that is considered a potential hoking hazard. Evidence: The Infant 2 classroom has children ages 1 year to 16 months. The lunch served today were whole chicken nuggets and sliced honeydew that is considered a potential choking hazard. Plan of Correction: We will instruct staff to cut food up in small bites to prohibit a choking hazard.
Standard #: 22VAC40-191-40-D-1-A Description: Based on staff interviews and review of staff records, background checks were not obtained for a new person designated as applicant. Evidence: Staff #1 is a Board Officer. No background checks have been completed on this individual. All background checks must be completed by the end of the 30 days after the change. Plan of Correction: The fingerprint search, central registry search for Virginia and out of state and Sworn statement will be completed and forwarded to the licensing inspector within 10 working days.
