A Subsidy Health and Safety inspection was conducted on 11/19/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed 11/19/18. There were 47 children present in six classes. Five children's records were reviewed at the center. Five staff records were reviewed at the central staff record location and the inspection was completed there. Two medications were observed for one child in care. The inspector arrived at the center at 9:30 am and departed at 11:40 am. The staff record review began at the central location at 12:00 noon and concluded with the exit of the inspection at 4:15 pm. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-580-F Description: Based on record review and interview, the center failed to ensure that all staff who work directly with children completed first aid and CPR training within 30 days of employment. Evidence: 1. Staff #5 who was hired 6/15/18, terminated 8/3/18 for summer employment only did not have first aid and CPR within 30 days of employment. Staff #5 was rehired 11/19/18 and was observed working during the inspection. Staff #5 did not have current training or certification in first aid or CPR. Plan of Correction: According to the HR administrative staff person, staff #5 will complete first aid and CPR training within 90 days.
Standard #: 22VAC40-665-780-A-3 Description: Based on interview, the center failed to ensure that a lock down drill was practiced annually. Evidence: 1. According to interview with the site coordinator, a lock down drill had not been practiced. Plan of Correction: The site coordinator will ensure that at least one drill is practiced each year.
