A Subsidy Health and Safety inspection was conducted on 11/27/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was completed on 11/27/18. There were 87 children present in five groupings. Nine children's records and five staff records were reviewed (four additional staff records were reviewed for completion of background checks only). Seven medications were observed. The inspector arrived at the center at 10:30 am and departed at 5:00 pm. The children were observed during free play time in the classrooms, during lunch, during nap, during outside time, and during departures. All infant activities were also observed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-580-E-2 Description: Based on record review and interview, the center failed to ensure that all staff had first aid certification within 30 days of employment. Evidence: 1. There was no documentation in the record of staff #3 that the staff person had first aid training and certification within 30 days of employment. Staff #3 was hired 8/14/18, was suspended 8/15/18-10/16/18, and returned to work on 10/17/18. 2. According to the director, the staff person does not have first aid certification. Plan of Correction: The staff person will attend a first aid class this week.
