Description: Based on observation, areas of the center inside were not maintained in a safe and operable condition. Evidence: 1. In the Three Year old Classroom, the paint was peeling under the shelf by the window on the far left of the wall. 2. In the Four Year old Classroom, the carpet near the window had an exposed large L shape rip. 3. In the Pre-K Classroom located in the Wilkins building, the paint was peeling under the Circle Time bulletin board to the right of the bench.

Plan of Correction: The two rooms will be repainted and the carpet square will be replaced.

Description: Base d on observation, the center failed to cover all unused electrical outlets with protective covers. Evidence: 1. In the Four Year old classroom on the lower level, one unused outlet under the letter R was not covered.

Plan of Correction: I covered the outlet immediately.
