The sworn disclosure statement, fingerprint check, and Child Protective Services search of the central registry are all valid for five years from the original date conducted.
An unannounced monitoring inspection was conducted with the center director and center staff to determine compliance with the licensing standards. There were 11 four year old children and 16 three year old children with seven staff actively supervising. The four year olds were listening to a story while the threes were learning about the letter "D" and number 5. The children had a "red carpet" presentation before lunch. Lunch served today: corn dogs, mixed vegetables, grapes, and milk.
The last health inspection was conducted 7/11/2018. The last fire inspection was conducted 6/28/2018. The last shelter in place drill was conducted 9/26/2018. Three children's files and three staff files were reviewed and are in compliance. Two medications were reviewed - please see Supplemental Summary for missing items. Standard #: 22VAC40-185-510-A Description: Based on review of two emergency medications, the facility failed to have documentation of written parental authorization to administer the medicine to two children. Evidence: there was no written parental authorization for the facility to administer emergency medication to Child 1 and Child 2. There was no written physician authorization for the facility to administer medicine that is required for more than 10 days. Plan of Correction: Center director will get the required written parental and physician authorizations.
Standard #: 22VAC40-185-510-C Description: Based on review of two emergency medications, the facility failed to prevent the use of outdated medication. Evidence: the emergency medication on site for Child 2 expired 8/2018. Plan of Correction: Center director will request a current medication from the parent.
