Comments:
An unannounced renewal inspection was conducted today from 10:15am-1:30pm. There were 50 children (2-5yrs.) supervised by 7 direct care staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed engaged in circle time, playing with sensory tables, exploring shadows, and participating in various gross-motor activities. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, 1 of 5 children's records did not contain all of the required information. Evidence: Child #1's (start date: 9/5/2017) most recent documented parental acknowledgement of review of emergency information was dated 8/31/2017.

Plan of Correction: Parent contacted and provided hard copy update form to document prior online review of information on 11/29/18. Will ensure form is on file by 12/7/18.

Standard #: 22VAC40-185-510-F
Description: Based on a review of medications, 1 of 6 medications was not kept in the original container with the direction label attached. Evidence: Child #5's (start date: 9/4/2018) Children's Benadryl was not in the original container with the direction label attached.

Plan of Correction: Parent contacted on 11/29/18 to provide either original box for unopened labeled Benadryl bottle or a new box of Benadryl. Will ensure on site by 12/7/18.

Standard #: 22VAC40-185-510-J
Description: Based on a review of medications, 1 of 6 medications was not kept in the original container with the direction label attached. Child #5's (start date: 9/4/2018) medications were found in an unlocked plastic container in the teacher's outdoor travel bag.

Plan of Correction: Teacher immediately placed Epi-Pen in the locked cabinet. Safe-T-Sak container bag or locked box that can travel with group will be provided by the end of day.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, 1 of 3 staff records did not contain documentation of out-of-state central registry requests by the end of the 30th day of employment, and follow-up 45 days after the request to search. Evidence: 1. Staff A's (date of hire: 8/6/2018) sworn disclosure statement (completed 7/26/2018) indicated that the employee has lived in the state of Maryland within the past 5 years. 2. Staff A's record contained documentation of a Maryland central registry request sent 10/1/2018, which is past 30 days of employment. 3. Staff A's record did not contain any documentation of follow-up performed 45 days after the central registry request was sent.

Plan of Correction: Follow up phone call will be placed by end of day (11/29/18) to confirm clearance in process or cleared. Any steps necessary to resolve any technical issues with receiving the form will be done immediately.
