Technical Assistance:
Directors and inspectors discussed medications, transcripts, new subsidy requirements, and background checks.

Comments:
An unannounced monitoring inspection was conducted today from 12:45pm-3:10pm. There were 151 children (infants-5yrs.) directly supervised by 17 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, reading quietly, playing with drawing, coloring, tummy time with infants, circle time, infants in the buggy, art class, and dancing. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not ensure each child enrolled had a physical examination by or under the direction of a physician. Evidence: Child 1 is missing documentation of physical examination without 30 days of start date. (Child 1, Start Date 8/2017)

Plan of Correction: We will contact the family immediately regarding the documentation of physical examination.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center did not maintain all equipment in the center in a safe condition. Evidence: In the infant room, the clear protective plastic layer on the high chair's tray table was peeling and accessible to children The peeling plastic could potentially be picked up by an infant while eating, which poses a potential health hazard to the infants.

Plan of Correction: The center has reached out to the manufacturer to order a new tray table for the high chair.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method. Evidence: In the art room, hair spray and spray paint were kept in an upper unlocked cabinet.

Plan of Correction: The items were immediately placed in a locked cabinet preventing access to children.

Standard #: 22VAC40-185-570-E
Description: Based on observation, the center did not ensure all bottles were labeled with the date. Evidence: Two bottles were observed with no date label.

Plan of Correction: We will make sure to label those bottles.
