A Subsidy Health and Safety inspection was conducted on 11/30/18 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted from 8:15am-9:45am. Upon arrival there were 23 children in care with 3 staff members. A sample of 5 children and 3 staff records were reviewed in addition to 1 medication. Children were observed doing puzzles and getting their breakfast. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Standard #: 22VAC40-665-810-D-5 Description: Based on a record review and interview the vendor did not ensure staff had a list of names of the children with allergy care plans. Evidence: Staff did not have a complete list of children with allergy care plans. Child #5, who had a diagnosed food allergy, was not on the list kept by staff. The director stated there was another sheet, but was unable to locate it. Plan of Correction: The following corrective action was provided by the vendor: The second half of the list will be located and an extra one will be kept with the program files.
