An unannounced monitoring inspection was initiated on October 16, 2018 and completed on December 3, 2018 in response to a self-report made by the facility. Licensing Inspector reviewed facility documentation, interviewed staff and conducted classroom observations.
- Violations:

Standard #: 22VAC40-185-410-1 Description: Based on interview and review, facility did not ensure staff do not commit the forbidden action of physical punishment. Evidence: On September 28, 2018, staff #1 "flicked" child #1 on his forehead in response to this child and another child not being quiet for nap time. 1. Facility documentation completed by staff #1 states "(child #1) and (another child) were making noises on their beds. I approached (child #1) and flicked him on his forehead. After I flicked him, I apologized to (child #1)." 2. Interview with staff confirmed staff #1 was seen and heard approaching and kneeling down to two children in that area of the room telling them to lay down and be quiet. Plan of Correction: Upon report of the incident, staff member was questioned and removed from working on the floor. Staff member was kept off the floor until the internal investigation was completed. Staff #1 no longer works for Newport News Youth Programs.
