Thank you for your assistance during this unannounced inspection. This on-site visit was conducted on November 13, 2018 from 1:30 p.m. through 2:45 p.m. This inspection was initiated from a complaint that was received by the Valley Licensing Office on October 31, 2018 regarding the allegation of supervision. Interviews were conducted with three staff, one children's record and the attendance sheet was reviewed. The information gathered during this inspection supports the allegation, therefore the complaint is valid. Areas of non-compliance may be found on the violation page.

Standard #: 22VAC40-185-340-A Complaint related: Yes Description: Based on three staff interviews, the center failed to ensure the care, protection, and guidance of one child. Evidence: 1. On October 25, 2018, twenty children, ages five through ten, were in the care of one staff member in the school gymnasium waiting to be transported to the after-care program. Staff #1 stated the second staff member "had called in sick and he was sent alone to pick up the after school children." 2. Staff #1 noticed several children running around on the stage in the gymnasium. 3. Staff #1 told the children "two or three times to stop running around and get down from the stage." 4. Child #1(age 10) was being pulled on the straps of her backpack by another child. 5. At approximately 2:25 p.m., Child #1 fell down and chipped her two front teeth. Plan of Correction: Classroom management strategies are being taught again to the staff. Safety procedures are being trained to staff.
Standard #: 22VAC40-185-580-L Complaint related: Yes Description: Based on the review of one child's record, staff interview and review of policies and procedures, the center failed to have documentation of parental permission to transport children. Evidence: 1. Child #1's record was reviewed. There was no documentation of parental authorization for the center to transport the children. 2. Staff #3 stated "they used to have that in their handbook, but it had recently been updated." 3. Nineteen children were transported to the after school care on October 25, 2018 without documentation of written parent authorization. Plan of Correction: Parent permission to transport children from school will be included in the handbook.
