An unannounced Subsidy Health and Safety Inspection was conducted on December 5, 2018 from approximately 10:45 a.m. until 1:05 p.m. Upon entrance there were 6 children and 3 staff present with children. Additional staff arrived during the inspection. The record review sample consisted of 5 children's records and 5 staff records. Children were observed coloring, eating lunch and preparing for rest period. The results of the inspection were reviewed and verified with the center director. Standard #: 22VAC40-665-520-B-11 Description: Based on documentation review and interviews, the vendor did not ensure a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: During the inspection on December 6, 2018, the record for child #2 did not include the written allergy plan from the physical and child #2 has a diagnosed food allergy to carrots and milk. Plan of Correction: The center director will give mom a certain number of days to return the document for the allergy care plan.
Standard #: 22VAC40-665-580-F Description: Based on record review and interviews, the vendor did not ensure all staff who work directly with children and who are employed by an approved vendor must complete CPR and first aid training within 90 days. Evidence: During the inspection on December 5, 2018, Staff # 5 date of hire July 30, 2018 did not include certification for CPR or First Aid. Plan of Correction: The center director will have the staff member take the course.
Standard #: 22VAC40-665-770-A Description: Based on documentation review and interviews, the vendor did not ensure the emergency plan included all required procedures. Evidence: During the inspection on December 5, 2018, the evacuation, shelter -in-place and lockdown procedures did not include accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during lockdown, shelter-in-place, evacuation or relocation. Plan of Correction: The center director will update the emergency plan.
Standard #: 22VAC40-665-770-B-3 Description: Based on documentation review and interviews, the vendor did not ensure the Lockdown procedures to include facility containment had all required procedures. Evidence: During the inspection on December 5, 2018, the lockdown procedures did not include the designated lockdown locations and methods for accounting for all children. Plan of Correction: The center director will update the emergency plan.
