22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.

Comments:
An unannounced monitoring visit was conducted today from 2:00pm to 3:30pm. At the time of entrance, 32 children were in care with 2 staff members present. The physical space, emergency supplies, evacuation drills, attendance records, medications, the Emergency Preparedness Plan, 5 children's records and 2 staff's certifications for CPR, First Aid and MAT were reviewed. Ratios were in compliance and required documents were posted. The children were observed during check in procedures, playing games and playing on the computer. Standard #: 22VAC40-185-60-A
Description: Based on record review, all five children's records did not have complete information. Evidence: 1. The staff were unable to access their computer system with the children's records. 2. The center was missing all required information except the following: Child #1 had the child's Sex, both parent's names, one parent address, both parent's phone numbers and one parent's work phone number. Child #2 had the child's Sex, both parent's names, one parent address, both parent's phone numbers, both parent's work phone number, two emergency contact names, two emergency contact phone numbers and start date. Child #3 had the child's Sex, one parent's name, one parent address, one parent's phone number, two emergency contact names, and two emergency contact phone numbers. Child #4 had the child's Sex, both parent's names, one parent address, both parent's phone numbers, one parent's work number and start date. Child #5 had the child's Sex, both parent's names, one parent address, one parent's phone number and start date.

Plan of Correction: I will request a paper copy of the children's records.

Standard #: 22VAC40-185-550-E
Description: Based on review, the center failed to maintain a record of the dates of the practice drills for one year. Evidence: 1. The center had record of practice drills for the month of August 2018 through November 2018, but did not have record of the drills practiced February 2018 through June 2018.

Plan of Correction: I will request a copy from the office and will make sure I keep 12 months of practice drills on site.
