Discussed with center director: Background checks for staff, Safety training certifications, the appropriate use of equipment used by children.

An unannounced non-mandated monitoring inspection was conducted today from 11:15am to 1:15pm. The purpose of this visit is to assess previous violations. There were 50 children present with 13 staff members. The children were observed playing outside, playing with toys, washing hands and eating lunch. The lunch menu included pasta with mixed vegetables and milk. 5 staff records and 4 children records were reviewed. Emergency drill records were current. Description: Based on observations, areas and equipment, inside and outside the center were not maintained in a clean and safe condition. Evidence: 1. Children were observed climbing over and into one of two sensory tables on the playground. The sensory table had two openings for bins. The bins were not in the table when the children were observed climbing into and over the table. The lids for the bins were found on a different part of the playground. 2. The lower shelf of a white toy shelf in the Preschool room was observed to be dirty with grime. 3. There is visible dirt in corners on the floor and around the baseboards in classrooms. It appears to be dirt that may have accumulated over time.

Plan of Correction: The sensory tables not in use will be removed to a different part of the playground. The shelf will be cleaned. A maintenance request will be put in today for a deep cleaning. The request will be put in today.
