Today an unannounced monitoring inspection was conducted between the hours of 3:15pm and 5:00pm. I observed 61 children directly supervised by 4 staff. Ratios and supervision were in compliance. Children were observed playing outside, playing with legos, coloring, doing homework, doing craft, and participating in group play. Staff were actively engaged with the children and assisting them with tasks as needed. The physical plant, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Required staff training documents were on-site. Emergency preparedness supplies in order. Required documents posted. Record keeping was in compliance. Questions about this inspection may be directed to: firstname.lastname@example.org
Standard #: 22VAC40-185-510-N Description: Based on review of medication and interview, medication with an expired authorization form was not returned to the parent within 14 days of expiration. Evidence: The medication authorization form for a prescription allergy medication for child#1 expired on 6/30/2018. The medication was observed on-site during the inspection. Plan of Correction: We will request a new authorization form again.
