Comments:
An unannounced subsidy health and safety inspection was conducted with the director and assistant director beginning at 1:00 PM and ending at 3:25 PM. There were eight 3 year old children in the care of two staff members. The children were observed playing with play dough, dolls, building blocks, and cars. Ten children's records were reviewed. Four staff records were reviewed. There were no medications on site to be reviewed. The emergency preparedness plan was reviewed and in compliance. The emergency escape drill log was reviewed. The most recent health inspection was documented as 4/13/18. The most recent fire inspection was documented as 2/13/18. Description: Based on a review of documentation and staff interview, the vendor did not conduct a practice lockdown drill as required. Evidence: There was no documentation of a lockdown drill being conducted.

Plan of Correction: The center recently had training regarding active shooter scenarios. The center will conduct an annual lockdown drill and document it on the practice drill log.
