An unannounced monitoring inspection was conducted today December 7, 2018 to determine the facilities compliance with licensing standards. The inspection was initiated at 9:00 am and concluded at 10:20 pm. At the time of the inspector's arrival children were having music and movement, dancing to and following instructions to a music tape. Later children worked in their journals, drawing a picture of anything they wanted and then dictating a story about their picture to staff. Children were divided into to smaller groups with one group working on counting with number cards matching gingerbread men with the coordinating number. The other group reviewed colors while playing a dramatic play activity. Children will start specials (art, library, music and gym class) in January. Staff were observed interacting with the children in a positive manner. The classroom is equipped with age appropriate materials and supplies. Children play on the grass and blacktop area when outside. The first aid kit and non-medical emergency supplies were inspected and found complete. All required information was posted on the parent bulletin board to include the License, last inspection notice, emergency numbers, daily schedule, evacuation maps, allergy list and menu. The inspector interacted with staff and children in care. Three staff records and five children's records were reviewed. Medications and medication authorizations were reviewed. Staff on site have current MAT certification, CPR, First aid and daily health observation training. The most recent evacuation drill was documented on 11/13/2018. The two required shelter-in-place drills were conducted on 09/27/2018 and 10/18/2018. Health Inspection 01/31/2018 Fire Inspection 02/23/2018 Today's Ratio: thirteen children with three staff. Violations cited during the licensing period were checked for corrections. There were no repeat violations cited. Standard #: 22VAC40-185-60-A Description: Based on review of children's records, the facility did not obtain all required information for each child's record. Evidence: 1. The record of child #1, enrolled 09/05/2018, did not have the address of the second emergency contact listed. 2. The record of child #4, enrolled 09/04/2018, did not have the address of the second emergency contact listed. Plan of Correction: Per the Administrator: Parents will be contacted to obtain missing addresses.
Standard #: 22VAC40-185-70-A Description: Based on review of staff records, the facility did not obtain all required information for each staff record. Evidence: 1. Staff #3, who was present during the inspection on 12/07/2018, did not have the name, address and phone number of a person to be contacted in an emergency on site at the center. 2. The record of staff #3 did not have written documentation of orientation training. The administrator reported staff #3 had been trained in orientation on 11/28/2018 but did not have the written documentation. 3. The record of staff #3 did not have documentation of two or more references. Plan of Correction: Per the administrator: 1. corrected, The emergency contact information was brought over by the administrator. 2. Orientation sheet will be sent for use. 3. HR will copy references form for the file.
