Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted from 9:30 am - 1:15 pm. There were 126 children in care being supervised by 20 staff members. The children were observed participating in story time, singing, and practicing for a holiday program. The infants were observed participating in gross motor skills and playing with soft toys on the carpet. Ten children's records were reviewed and in compliance. Eight staff records were reviewed. An allergy list was reviewed with children in the program with diagnosed food allergies. Allergy care plans were reviewed for those children with diagnosed food allergies. Staff records were reviewed for completion of CPR, First Aid, and Virginia Pre-Service Training. The most recent Lock down drill was conducted 06/05/2018. The facility has not yet complied with the following requirements: 22VAC40-665-650 Supervision, ratio, and group size requirements in regards to a written policy and procedure to ensure care by a consistent staff. *The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Standard #: 22VAC40-665-520-B-11
Description: Based on review of children's records it was determined that not all children with diagnosed food allergies had an allergy care plan with a physician's instructions on file. Evidence: The records for Child A, B, and C did not have documentation of an allergy care play signed by the physician on file.

Plan of Correction: Administration will request a signed allergy care plan for the parents of Child A, B, and C.

Standard #: 22VAC40-665-580-E-1
Description: Based on review of eight children's records it was determined that not all staff maintained current CPR training. Evidence: The most recent documentation of CPR training on file for Staff A was dated 09/02/2016.

Plan of Correction: Staff A will obtain current CPR training and submit documentation to administration.

Standard #: 22VAC40-665-580-E-2
Description: Based on review of eight children's records it was determined that not all staff maintained current First Aid training. Evidence: The most recent documentation of First Aid training on file for Staff A was dated 09/02/2016.

Plan of Correction: Staff A will obtain current First Aid Training training and submit documentation to administration.
