Comments:
The licensing inspector conducted an unannounced monitoring inspection on 12/04/2018 from 11:18am to 4:00pm. The children were observed playing with toys, playing outside, eating lunch and napping. The infants were observed being fed and napping in cribs. A formal observation was completed with the two year old and three year old classrooms. Lunch consisted of beef tacos with cheese, salad with ranch, pineapples and milk. The lunch menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Six children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Nine injury reports were reviewed as well as the center?s injury prevention plan which was updated 7/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/29/2018 Last shelter-in-place drill: 11/29/2018 Last fire inspection: 06/05/2018 Last health inspection: 02/05/2018 The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection. Standard #: 22VAC40-185-500-B
Description: Based on observation and an interview, the center did not ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The center's toddler classroom had an inoperable step trash can. The teacher stated that she was aware of the broken trash can and had informed the director of the broken step trash can the morning of the inspection.

Plan of Correction: Corrected during the inspection.

Description: Based on record review, the center did not ensure that four of eight staff records had documentation of central registry findings within 30 days of employment. Evidence: 1. The record of staff #3 (start date: 7/9/2018) had documentation of central registry results dated 08/10/2018. 2. The record of staff #4 (start date: 6/4/2018) had documentation of central registry results dated 07/23/2018. 3. The record of staff #6 (start date: 6/18/2018) had documentation of central registry results dated 07/25/2018. 4. The record of staff #8 (start date: 6/4/2018) had documentation of central registry results dated 07/23/2018.

Plan of Correction: 1. Staff # 3-The Criminal History request was mailed on 6/20/2018 (start date 7/9/18) and received back on 8/10/18. This staff member was not left alone with any children during that time period. 2. Staff #4- The Criminal History request was mailed on 5/29/2018 (start date 6/4/2018) and received back on 7/23/2018. This staff member was not left alone with any children during that time period. 3. Staff #6- The Criminal History request was mailed on 5/29/2018 (start date 6/18/2018) and received on 7/25/2018. This staff member was not left alone with any children during that time period. 4. Staff # 8- The Criminal History request was mailed on 5/29/2018 (start date 6/4/2018) and received on 7/23/2018. This staff member was not left alone with any children during that time period. Moving forward we will call the Office of Background Investigations Unit at the Department of Social Services for a status update on the report and document conversation within new hire?s first 30 days.
