An unannounced monitoring inspection was conducted this date. A total of 31 children with 2 staff present. Children were observed arriving for the after school program, sitting for attendance and lining up for hand washing before lunch. Clean up and sanitizing of table were observed. Some children chosen for leaders for the week. Children transitioned to book reading, coloring and engaging in small group activities. Children were also observed lining up to transition to the gym area for large gross motor activities. A total of 5 children's records, documentation of CPR/First Aid, MAT training and daily health observation was reviewed. All medications were reviewed and found complete with authorizations. The previous violations from the May inspection were reviewed and corrective actions were verified. Thank you for your cooperation today. This inspection began at 2:10pm and ended at 4:05pm. Standard #: 22VAC40-185-60-A Description: Based on child record review, one record was found incomplete. Evidence: Child # 1's proof of identity did not include verification that the child's identity and age was verified. Plan of Correction: We will obtain this information from the administrative office.
Standard #: 22VAC40-185-240-D-5 Description: Based on record review staff interviews, verification of at least one staff member on duty with daily health observation training was not found. Evidence: No documentation that the two taff on site today had completed daily health observation instruction. Plan of Correction: Both staff state that the training has been completed. Certificates will be obtained and maintained at the site.
Standard #: 22VAC40-185-340-B Description: Based on staff interviews, the center does not have an adult on the premises during the center's hours of operation in charge of the administration of the center. Evidence: Staff #1 and #2 stated that they have not be appointed in charge of the administration by the licensee. Staff stated that they have not had someone appointed in charge this school year. Plan of Correction: The licensee will appoint a staff designated in charge of the administration and will ensure that this staff is on the premises during the center's hours of operation.
Standard #: 22VAC40-185-560-F Description: Based on observation of the programming area, the menu posted was not current. Evidence: The snack menu posted was for the month of November. Plan of Correction: We will post the current December snack menu.
