63.2(17) License & Registration Procedures
An unannounced monitoring inspection was conducted between 10am and 12:30pm. There were 36 children present ranging in age from 18 months through six years old. There were seven teachers and two administrators present during the inspection. Children and staff records were reviewed. Children were observed in teacher and self-directed activities. Required postings, inspections, emergency evacuation drills, radio and flashlight were reviewed. Violations regarding tuberculosis screening, background checks, physical plant and over-the-counter diaper cream were identified and discussed with the program director at the conclusion of the inspection.
Standard #: 22VAC40-185-160-A Description: Based on record review and interview the center failed to ensure that each staff member submits documentation of a negative tuberculosis screening within 12 months prior to employing. Evidence: The record for staff 2 contains a tuberculosis screening that is dated 10/28/16. However, the documented date of hire for staff 2 is 4/10/18 and therefore, does not meet the requirement of having been completed within 12 months prior to employing. Plan of Correction: Staff 2 will be sent to obtain a current tuberculosis screening as soon as possible.
Standard #: 22VAC40-185-270-A Description: Based on observation and interview the center failed to ensure that areas are maintained in safe condition. Evidence: 1 - A radio is plugged in and sitting on top of a water fountain in the three to five year old room. Having an electric radio plugged in above a water fountain that is operational poses a shock hazard. a. Staff 4 confirmed that the radio was plugged in and sitting on top of the water fountain. 2 - There is a telephone cord dangling from telephone in the two year old room. The cord is dangling down within reach of children in such a way that it poses an entanglement hazard. a. Staff 5 confirmed that the telephone cord is dangling down within reach of children. Plan of Correction: Center management will ensure that areas are maintained in safe condition during all hours of operation The radio was removed to safe location during the inspection. The telephone will either be removed from the classroom or plugged in so that it does not pose an entanglement hazard.
Standard #: 22VAC40-185-520-C Description: Based on review of documentation and interview the center failed to maintain written authorization for the administration of over the counter diaper cream. Evidence: 1 - There is a tub of Destine diaper cream in the toddler room that does not have a written authorization to administer the cream. 2 - Staff 6 stated that she does not have a written authorization for the diaper cream. Plan of Correction: Center management will ensure that written authorizations are maintained for each diaper cream in the center.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and interview the center failed to obtain a central registry (CPS) finding within 30 days of employment. Evidence: 1 - The record for staff 2 has a CPS finding that is dated 2/27/17. However, the documented date of hire for staff 2 is 4/10/18. Therefore, the previously obtained finding is outdated. Staff 2 was present and providing direct care to children in the five and six year old classroom. 2 - The record for staff 3 does not contain documentation that any CPS finding was requested. The documented date of hire is 11/19/18. Staff 3 was present during the inspection. 3 - Staff 1 confirmed that there are not CPS findings on file for staff 2 or 3. Plan of Correction: Center management will place requests for CPS findings immediately. Within 10 days of today documentation that the requests have been placed will be forwarded to the licensing inspector. Once the results of the requests are received the information will be forwarded to the inspector within 10 days of receiving the information.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on record review and interview the center failed to provide fingerprinting for each staff as required. Evidence: 1 - The record for staff 3 does not contain documentation of fingerprint results. Staff 3 has a documented date of hire as 11/19/18. Staff 3 was present in the child care center during the inspection. 2 - Staff 1 confirmed that there is not any documentation of fingerprinting in the file of staff 3. Plan of Correction: Center management will ensure that all staff have documentation of fingerprint results on file prior to being allowed to work in the child care center.
