Comments:
An unannounced renewal inspection was attempted on Thursday, December 06, 2018 at 10:00am. The licensing inspector learned there were no children in care on this date and was unable to review the center's compliance with all licensing standards. The licensing inspector conducted an unannounced renewal inspection on Friday, December 07, 2018 from 10:00am to 11:50am. During the inspection, the center's director was present and assisted the licensing inspector. The census for today's inspection consisted of one child in care and 2 staff. During the inspection, the licensing inspector observed the child tracing letters, learning colors, playing with toys, and interacting with staff. The center is equipped with age and stage appropriate materials and supplies. Staff were observed having positive interactions with the child. The following information was reviewed: program activities, supervision, staffing, and interaction with children, daily routines, posted information, drill records, and agency inspection reports. The last documented health inspection occurred August 02, 2018. The last documented fire inspection occurred August 03, 2018. The center does not administer medication and does not provide transportation at this time. The first aid kit and non-emergency supplies were inspected and found to be in compliance. There were two children's records and three staff's records reviewed during this inspection. Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of three staff records on12/07/2018, the center did not ensure the results of a fingerprint background check were completed for two staff, hired prior to January 22, 2018, by September 30, 2018. EVIDENCE: 1) The record for Staff #1 (DOH: 08/08/2015) did not contain the results of a fingerprint background check. 2) The record for Staff #2 (DOH: 07/20/2015) did not contain the results of a fingerprint background check.

Plan of Correction: Per the Center: Now that I have the information for the fingerprint background. I will do it ASAP.

Description: Based on record review on 12/07/2018, the center did not ensure the results of a fingerprint background check were completed for one applicant/licensee, hired prior to January 22, 2018, by September 30, 2018. EVIDENCE: 1) The record for Applicant #1 did not contain the results of a fingerprint based background check.

Plan of Correction: Per the Center: Now that I have the information for the fingerprint background. I will do it ASAP.
