New subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirement: Continuity of care. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

A Subsidy Health and Safety Inspection (SHSI) was conducted on (insert date) to review supplemental health and safety requirements. An unannounced inspection was begun in response to the receipt of a self reported incident in our office. A joint investigation was conducted with local authorities. Based upon a review of documentation and multiple staff interviews, violations were cited. See Violation notice for further details. Staff member A declined an interview request. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. No violations of the supplemental subsidy health and safety requirements were found.
