Comments:
An unannounced mandated Monitoring inspection was conducted today from 10:15 to 12:30 pm. There were 51 children present with 12 staff. Children were observed outside on the playground, participating in gross motor activities, table tray activities, snack, singing songs, and reading books. Reviewed 5 staff files, 3 children's files, emergency evacuation procedures and log, fire inspection, playground and physical plant. Violations of the Standards are recorded on the Violation Notice. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, a child did not have a documentation of a physical exam within 30 days of the first day of care. Evidence; Child#1, start date 9/4/18, did not have a physical within 30 days of the first day of care.

Plan of Correction: I will ask the parent for it

Standard #: 22VAC40-185-280-H
Description: Based on observation, purses were stored in areas accessible to children. Evidence: 2 staff purses were observed stored in unlocked closets and on shelves accessible to children. This violation was corrected at the time of this inspection.

Plan of Correction: I will tell staff to store purses in a locked cabinet or on the very top shelf.
