An unannounced monitoring inspection was conducted today with the director who was present with 29 children and three additional staff. The children were on their cots for nap time during the inspection. One child had medication on site. Five staff and children's files were reviewed today. Arrival time: 12:30 pm Departure time: 3:20 pm

Standard #: 22VAC40-185-270-A Description: Based on observation, it was determined that the outside area was not maintained in a safe and operable condition. Evidence: Two boards on the fence surrounding the playgroud were in disrepair- protruding out from the fence and had nails exposed. Plan of Correction: I will nail the boards.
Standard #: 22VAC40-185-280-B Description: Based on observation, it was determined that hazardous substances were not kept locked. Evidence: In an unlocked cabinet in the Pre-K classroom was a container of air freshener spry and hand sanitizer. Plan of Correction: We had a lock for the cabinet but forgot to lock it back. I will make sure to keep the cabinet locked.
Standard #: 22VAC40-185-330-B Description: Based on observation and an inspection of the playground, it was determined that the preschool playground with the three portable slides had insufficient mulch under and around the equipment. Evidence: There was 3 inches of mulch underneath and surrounding the three portable slides on the preschool playground. The standard requires at least 6 inches of mulch. Plan of Correction: We will place the slides off limits until we can add the required mulch.
Standard #: 22VAC40-185-500-B Description: Based on observation , it was determined that the diapering area in the younger toddler room was not used for diapering only. Evidence: The diaper pad in the younger toddler room was used as a storage area for toys- basketball. Plan of Correction: I will talk with the teachers about not storing items on the diaper pad.
Standard #: 22VAC40-185-510-C Description: Based on an inspection of the medication for one child and an interview with the director, it was determined that parental and physician authorization for the Ventolin Albuterol Aerosol had expired on 7/8/18 and the authorization had not been renewed. Evidence: Parental and physician authorization had an effective until date of 7/8/18 for the Ventolin Aerosol for child A. The authorization has not been renewed nor has the medication been returned to the parent or disposed of. Plan of Correction: I will return the medication to the parents and have them and the physician update the authorization.
Standard #: 22VAC40-185-550-M Description: Based on a review of injury/accident reports for 11 children, it was determined that 7 of the 11 did not have the time parents were notified of injuries to their child nor was their a second signature. Plan of Correction: I will retrain the staff.
