Comments:
An unannounced monitoring inspection was conducted on 12/14/18 from 9:25am to 12:50pm. At the time of entrance there were 70 children ages five months old to five years old in care with 17 staff. Children were observed participating in various activities in the classrooms, eating snack, and during rest period. Records were reviewed for five children and five staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-340-D
Description: Based on observation, a review of five staff records and interviews, it was determined that the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present. Evidence: 1. There were 3 staff working in the Infant classroom with 9 children. None of the records for the staff contained documentation to demonstrate that any of the staff were Program Leader qualified. 2. Staff #8 (Program Director) reviewed the records for the staff #1in the Infant classroom, and confirmed that none of the records contained documentation to demonstrate that any of the staff were Program Leader qualified.

Plan of Correction: The facility responded: We will ensure that all lead teachers have the required training to be qualified as a Program Leader.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interviews, it was determined that the licensee did not ensure that disposable diapers shall be disposed of in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system during disposal. Evidence: 1. The Licensing Inspector observed a soiled diaper laying in a cardboard box outside the door to the Infant classroom. T 2. Staff #8 (Program Director) confirmed that the soiled diaper was not disposed of in a leakproof or plastic-lined storage system.

Plan of Correction: The facility responded: Staff will be re-trained on disposal of soiled diapers in a leakproof or plastic-lined storage system. center management will monitor to ensure compliance.

Standard #: 22VAC40-185-510-G
Description: Based on a review of the medication being stored at the facility, it was determined that the licensee did not ensure that all prescription medication is dispensed from a pharmacy and maintained in the original, labeled container. Evidence: 1. The medication (Epi-pen) for child #4 was not in the original, labeled container. 2. Staff #8 reviewed the medication for child #4, and confirmed it was not in the original, labeled container.

Plan of Correction: The facility responded: The medication will be given back to the parents of child #4, and instructions will be given that it will only be accepted if it is in the original, labeled container. All new medication will only be accepted if it is in the original, labeled container.
