Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on December 12, 2018 from 11:45 a.m. through 3:15 p.m. There were one-hundred twenty three children in the care of twenty staff. Ten children's and ten staff records were reviewed and updated. The medication and medication paperwork was reviewed for four children. Areas of non-compliance may be found on the violation notice. Areas observed were: free choice activities, hand washing/bathroom and diapering procedures, lunch, and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Standard #: 22VAC40-185-130-B Description: Based on record review the center failed to obtain an updated immunization for one child. Evidence: 1. Seven children's records were reviewed. 2. Child #2 did not have documentation of obtaining additional immunizations every six months. The center had documentation of requesting an update. 3. The director confirmed an update for Child #2 had not been obtained. Plan of Correction: The parents will be contacted and an update of additional immunizations will be requested.
Standard #: 22VAC40-185-160-C Description: Based on record review, the center failed to obtain an updated tuberculosis screening for one staff member. Evidence: 1. Ten staff records were reviewed. 2. Staff #10 di did not have documentation of obtaining an updated tuberculosis screening. 3. Staff #10's last documented tuberculosis screening was dated 12/05/2016. Plan of Correction: Staff #10 will obtain an updated tuberculosis screening.
Standard #: 22VAC40-191-40-D-1-B Description: Based on record review, the center failed to maintain a central registry search for one employee within 30 days of employment. Evidence: 1. Ten staff records were reviewed. 2. Staff #5 did not have documentation of a central registry search. 3. The director confirmed a current central registry search had not been obtained. Plan of Correction: An updated central registry search will be obtained immediately.
