Licensing inspector discussed the importance, responsibility, requirements and duty of a mandated reporter of suspected and/or alleged child abuse and neglect with the administrative staff. Internal investigations should not be conducted until after allegations are reported to and investigated by the appropriate agencies.
An unannounced investigation was conducted 11/01/2018 in response to a self reported incident regarding supervision. During the investigation, staff were interviewed, staff and children's files were reviewed and rooms 12 and 35 were observed. The preponderance of evidence does not support the self reported incident; therefore, no violations were cited related to the self-reported incident. The exit interview was conducted with the director. If you have any questions pertaining to this investigation, please contact Kimberly.Sawyer@dss.virginia.gov.
A compliance history is in no way a rating for a facility.
The online compliance history includes only information after July 1, 2003. In addition, the online compliance history includes information regarding adverse actions that may be the subject of a pending appeal. An adverse action is not final until a provider has exhausted or waived all due process rights. For compliance history prior to July 1, 2003, or information regarding the status of pending adverse actions, please contact the Licensing Inspector listed in the facility's information. The Virginia Department of Social Services (VDSS) is not responsible for any errors in or omissions from the compliance history information.
