Licensing inspector discussed the importance, responsibility, requirements and duty of a mandated reporter of suspected and/or alleged child abuse and neglect with the administrative staff. Internal investigations should not be conducted until after allegations are reported to and investigated by the appropriate agencies.

An unannounced investigation was conducted 11/02/2018 in response to a self reported incident regarding supervision. During the investigation, staff were interviewed, staff and children's files were reviewed and the infant and toddler rooms were observed. The preponderance of evidence does not support the allegation; therefore, no violations were cited related to the self-reported incident. A violation not related to the reported incident was found. Please see violation notice for details. The exit interview was conducted with the director. Standard #: 22VAC40-185-160-C Description: Based on record review, each staff did not resubmit tuberculosis test results at least every two years. Evidence: Staff B did not submit TB test results at least every two years. TB test results were last completed 05/27/2016. Plan of Correction: Corrected by the 8th of November. This was viewed by the Licensing Inspector during the semi-annual file review.
Standard #: 22VAC40-185-80-B Description: Based on interviews with staff, a staff person failed to report an alleged case of child abuse. Evidence: 1) Staff A stated that in April 2018, Child A alleged that Staff B hit him. Staff C stated that an internal investigation was conducted into the allegation. 2) The suspected abuse was not reported to the local department of social services in accordance with ? 63.2-1509 of The Code of Virginia. 3) Staff C stated "I didn't think this was a reportable offense because based on the internal investigation I did not believe it was founded." Plan of Correction: Agency reviewed Mandated Reporter Policy with all staff per VDSS guidelines and internal procedures on Monday, 12/3.
