22VAC40-185 SPECIAL CARE PROVISIONS AND EMERGENCIES.
An unannounced monitoring inspection was conducted with the director beginning at 3:45 PM and ending at 6:00 PM. There were 55 school-age children in the care of 4 staff members. The children were observed during outside recreational time. Hand washing and bathroom procedures were observed. Children were observed eating snack which consisted of cheese crackers and fruit juice. The children were also observed playing games, reading and doing homework. Ten children's records were reviewed and in compliance. Four staff members' records were reviewed and in compliance. Fifteen medications were reviewed. The emergency escape drill log was reviewed and in compliance. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent health inspection was dated 6/5/18. The most recent fire inspection 11/7/18. Standard #: 22VAC40-185-340-F Description: Based on observation, the staff did not always maintain actual sight and sound supervision of all children under the 10 years old. Evidence: On December 11, 2018 at approximately 4:30 PM the Licensing Inspector observed three children out of staff sight and sound supervision. Staff #1 was standing at the door between the gym and cafeteria while the children while the children went into the cafeteria's kitchen area to get the snacks. The door to the kitchen area was closed while the children were inside the kitchen area. Plan of Correction: Staff members will be re-trained regarding maintaining sight and sound supervision.
Standard #: 22VAC40-185-510-C Description: Based on a review of documentation, the center did not obtain current written authorization from the child's physician and parent for a long term medications prescribed to children in their care. Evidence: The medication authorization for Child A's two long term medications had expired on 11/23/18. Plan of Correction: The center will obtain a ten-day authorization from the parent until a long-term authorization can be obtained from the child's physician.
