Comments:
A monitoring inspection was conducted with the director and one staff. Five children were in care and the age range was 12 years to 13 years old. Children were observed during snack time, free play and homework time. All the required paper work was posted. No children were on medication at this time. The inspection time was 3:15pm-4:20pm.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on review of three staff records not all were complete. Evidence: Staff A with a hire date of 8/25/18 only had one references and it was not signed by the person that checked it.

Plan of Correction: I will check with the main office.

Standard #: 22VAC40-191-40-D-1-A
Description: Based on review of three staff records not all had a search of central registry with in 30 days of employment. Evidence: Staff A with a start date of 8/25/18 and staff B with a start date of 8/27/18 did not have documentation of a search of central registry check.

Plan of Correction: I will check with the main office.
