63.2(17) License & Registration Procedures
A monitoring inspection was conducted on October 18 and November 29, 2018 with a representative of Child Protective Services in response to a self reported incident received by the Fairfax Licensing Office on October 15, 2018. The inspection consisted of interviews with center and school staff and a review of the child care area and student records. Violations are listed in the violation notice. Standard #: 22VAC40-185-340-F Description: Based on interviews with SACC and school staff, the child day program did not ensure that children under 10 years of age were always within actual sight and sound supervision of staff. Evidence: 1. On October 15th the center self reported that at approximately 4:55-5:00pm in the afternoon of October 12, 2018, Child 1 (eight years of age) was able to walk from the playground, which is fenced but not enclosed, and into the road in front of the school where two community members observed the child in the road and escorted the child to the school's main office. 2. Staff 1 and 2 observed Child 1 playing on the playground slide and bridge at approximately 4:45pm. Staff 1, Staff 2, and Staff 3 were alternately observing children on the playground and taking groups of children to the bathroom. 3. Staff did not see Child 1 exit the playground or notice the child was missing. 4. At approximately 5:00pm Child 1 was escorted by two community members to the front office and the SACC staff were notified that Child 1 was in the office. Plan of Correction: All center staff will ensure that children are within sight and sound supervision at all times. The Center Supervisor, with the input of staff, will review and update the outside supervision plan to ensure the best procedures are in place and incorporate both staff and child transitions. The plan will be reviewed with the Regional Supervisor, and with all center staff including substitute teachers. Additional supervision training will be conducted for center staff by SACC Regional Supervisors.
