Technical Assistance:
Emergency Preparedness plan assistance, and supervision techniques.

Comments:
An unannounced mandated Monitoring inspection was conducted today. There were 147 children with 32 staff present. Children were observed painting, singing songs, having circle time and music time, outside on the playground, reading books, building with blocks, legos and colored pegs, playing with puzzles, drawing and coloring. Reviewed 8 staff files, 8 children's files, emergency preparedness plan, log and supplies, attendance, and physical plant. Violations of the Standards have been recorded on the Violation Notice. Violations:
Standard #: 22VAC40-185-40-K
Description: Based on observation, there was not active supervision in the nature classroom by staff including positioning of staff in strategic locations, scanning play activities, and circulating among children. Evidence: 4 children were observed running towards the drainage ditch with standing water multiple times without supervision. Children were observed standing on the drainage pipe and looking into the water. Children were re-directed away from the area when another staff member exited through a door by the drainage ditch and observed the children.

Plan of Correction: The staff involved will be written up and re-trained.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas accessible to children were not maintained in a safe and operable condition. Evidence: Multiple ceiling tiles were missing in areas that were accessible to children.

Plan of Correction: I will let the maintenance staff know.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the diapering areas in classrooms do not have a sinks with running warm water. Evidence: Toddler and Young Preschool classrooms did not have sinks in the diapering areas.

Plan of Correction: We will have them installed.
