Technical Assistance:
Technical assistance was provided regarding materials being used for the Montessori program, the renewal of the conditional license, staff trainings and background checks.

Comments:
An unannounced non-mandated monitoring inspection was conducted from 10:50am to 1:00pm. There were 5 children present with 4 staff members. The children were observed engaged in structured activities using a variety of learning materials. The children were also observed playing outside. The children had brought food from home that was found to be labeled. 5 children's records and 4 staff records were reveiwed. Areas of non compliance are listed in the violation notice. Standard #: 22VAC40-185-130-A
Description: Based on records review, the center did not obtain immunization records for each child enrolled before their first date of enrollment. Evidence: 1. Child # 1's first date of enrollment was 10/18/18. 2. Child # 2's first date of enrollment was 11/7/18. 3. Child # 3 and Child # 5's first date of enrollment was 10/11/18. 4. Immunization records were not obtained for any of the 4 children before they entered the center. Immunization records were not available for review.

Plan of Correction: We will remind the parents to bring in the immunization records.

Standard #: 22VAC40-185-140-A
Description: Based on records review, the center did not obtain documentation of a physical exam for each child within one month of attendance. Evidence: 1. Child # 1's first date of enrollment was 10/18/18. 2. Child # 2's first date of enrollment was 11/7/18. 3. Child # 3 and Child # 5's first date of enrollment was 10/11/18. 4. A physical exam was not obtained for any of the 4 children enrolled within one month of enrollment.

Plan of Correction: We will remind the parents to bring in the physical forms.

Standard #: 22VAC40-185-160-A
Description: Based on record review and staff interview, the center did not obtain tuberculosis (TB) screenings from each staff employed within 21 days of hire. Evidence: 1. Staff # 1 was hired on 11/19/18. Staff # 1's record did not have a TB screening in her record. 2. Staff # 1 stated that she has to schedule an appointment to get the TB screening completed.

Plan of Correction: Staff will make appointment for the TB screening.

Standard #: 22VAC40-185-60-A
Description: Based on records review, the center did not obtain proof of identification for each child enrolled. Evidence: 1. Child # 2's first date of enrollment was 10/11/18. The center did not verify proof of identification within 7 days of enrollment. 2. Child # 4's first date of enrollment was 9/17/18. The center did not verify proof of identification within 7 days of enrollment. 3. Child # 5's first date of enrollment was 10/11/18. The center did not verify proof of identification within 7 days of enrollment.

Plan of Correction: We will get the proof of ID from the parents as soon as possible.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the center did not obtain two references for an employee before the first date of employment. Evidence: Staff # 1 was employed on 11/19/18. Two references were not obtained before Staff # 1 started employment at the center.

Plan of Correction: We will get references done as soon as possible.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on record review, the center did not obtain fingerprint results for an employee before the first date of employment. Evidence: Staff # 1 was hired on 11/19/18. Staff # 1's fingerprint results were dated 11/30/18 indicating that the results were not obtained before the first date of employment.

Plan of Correction: We will remember to obtain fingerprint results before a new employee begins employment.
