An unannounced renewal inspection was conducted on December 18 2018 between 1:30pm and 4:00pm. There were 27 children observed in the direct care of 6 staff members. Children were observed napping, washing hands, preparing for snack, and eating snack. Staff were observed to be consistently engaged with the children. Standard #: 22VAC40-185-130-B Description: Based on record review, 1 of 5 children's records reviewed did not have documentation of additional immunizations once every six months for children under the age of two. Evidence: Child #1 record last documented immunization record is dated 05/02/2018. The child will not be two years of age until October 2019. Plan of Correction: I will contact the parent for the shot record.
Standard #: 22VAC40-185-70-A Description: Based on record review, 2 of 5 staffing records reviewed do not have the required documentation on record. Evidence: Staff #4 and Staff #5 records do not have documentation that supports the education requirements for their positions. Plan of Correction: One staff member will need to fill out an allowable variance and the other staff member will bring it in from home.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center did not keep hazardous substances in a locked place using a safe locking method. Evidence: in the school age room, an interior construction adhesive and interior wall paint can were located by the foosball table. The room was not in use at the time, but the items were not locked. Plan of Correction: The items were not in a used room but were locked up before children arrived.
