Technical Assistance:
During the inspection, the licensing representative discussed with the applicant: - terms of a conditional license - obtaining a variety of age appropriate materials and supplies for vacant rooms - staff qualifications and background check requirements - risk matrix [supplemental page] - model forms and injury reporting template

Comments:
An announced initial inspection of the facility was conducted today from 10:50am-1:05pm. No children were in care. Applicant background checks, 1 staff record, children's record templates, staff policies and procedures, physical plant, emergency supplies, and playground equipment were reviewed/inspected. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations, areas inside and outside of the center were not maintained in clean, safe and operable conditions. Evidence: 1. A door knob was missing on a door in a room intended for use by 2-3 year olds [Classroom 8] - posing an entrapment hazard. 2. Areas of rust were observed on two metal bathroom stall dividers between classroom 10 and 12. 3. 3 [approximately one inch wide] gaps were observed on the resilient surfacing on both the school age and infant/toddler playgrounds - posing a tripping hazard.

Plan of Correction: 1. Purchased and installed door hole cover plate. 2. Stall dividers will be cleaned and painted. 3. Gaps will be repaired using resilient surface repair material.

Standard #: 22VAC40-185-540-C
Description: Based on observations, the first aid kit did not include the required supplies. Evidence: The first aid kit was missing a thermometer and 1 of 2 required triangular bandages.

Plan of Correction: Thermometer and triangular bandages will be purchased.

Standard #: 22VAC40-185-540-E
Description: Based on observations, the required non-emergency supplies were not available during the inspection. Evidence: A battery operated flashlight was on site.

Plan of Correction: Flashlights have been purchased.
