63.2 Facilities & Programs.
An unannounced renewal inspection was completed today. The physical plant, 5 staff files, 7 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 1:30-3:00pm. There were 77 children under the supervision of 14 staff. During the inspection, children were observed being fed (infants), dancing to music, reading books, enjoying free play and resting during naptime. For lunch, the children were served italian turkey meatball sub, spinach, fruit and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 5 staff certified in MAT (medication administration training). Should you have any questions, please contact me at email@example.com.
Standard #: 22VAC40-185-130-A Description: Based on file review, 1 out of 5 children's records does not have documentation that each child has received the immunizations required by the State Board of Health before attending the center. Evidence 1. Child #1 does not have an immunization record on file. Plan of Correction: The Director believed that this had been received, but could not locate the paperwork. She contacted the parent during the inspection, asking for this to be brought to the center.
Standard #: 22VAC40-185-140-A Description: Based on file review, 1 out of 5 children's records does not have documentation of a physical examination. Evidence 1. Child #1 does not have a physical on file. Plan of Correction: The Director believed that this had been received, but could not locate the paperwork. She contacted the parent during the inspection, asking for this to be brought to the center.
Standard #: 22VAC40-185-270-A Description: Based on observations, the center failed to maintain all areas of the center in a safe condition. Evidence 1. In the Dolphin classroom, there were sections on the wall between the computer and emergency exit, where the paint is peeling off. Plan of Correction: The Director will have this fixed.
