The licensing inspector conducted an unannounced monitoring inspection on 12/18/2018 from 11:20am to 4:15pm. The children were observed playing with toys, playing on the playground, reading, eating lunch and preparing for nap. A formal observation was completed with the infant and two year old classrooms. Lunch consisted of turkey and cheese sandwiches, tropical fruit, carrots and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Eight children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Ten injury reports were reviewed as well as the center?s injury prevention plan which was updated 12/04/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 11/13/2018 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 08/02/2018 Last health inspection: 08/09/2018 Today, the following child to staff ratios were observed: Infants - 7:2 Toddlers ? 10:2 Twos #1 - 15:2 Twos and Threes - 15:2 Threes - 20:2 Fours ? 20:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-140-A Description: Based on review, the center did not ensure that one of eight children's records contained documentation of a physical examination within 30 days of attendance. Evidence: The record of child #3 (start date: 10/1/18) did not have documentation of a physical examination. Plan of Correction: I will notify the parent today that we need it and will secure the physical by Friday.
Standard #: 22VAC40-185-570-A Description: Based on observation, the center did not ensure that the protective belt was fastened securely when a child is placed in an infant seat or high chair. Evidence: The infant classroom had three infants eating lunch. Two infants were in high chairs and one infant was in a feeding chair that sat close to the floor. All three infants did not have protective belts fastened. Plan of Correction: I will make sure that all infant staff are aware that protective belts are buckled when children are in the high chair.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review, the center did not ensure that one of five staff records contained documentation of an out of state child abuse and neglect request by the end of the 30th day of employment. Evidence: The record of staff #1 (start date: 9/10/2018) did not have documentation of an out of state child abuse and neglect request. Plan of Correction: I will access the web site, fill out the form, and mail the form out by tomorrow.
