Technical assistance was provided regarding sending me a copy of the out of state background check when it is found or returned.
- Comments:

An unannounced monitoring inspection was conducted today from 10:00am-1:30pm. There were 50 children (infants-5yrs.) supervised by 11 direct care staff. The physical plant, programming, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, art, reading books, playing outside. Diapering and hand-washing procedures were also observed. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-160-C Description: Based on review of staff records, not all staff have documentation of a TB test every two years. Evidence: Staff #1 has a TB test that expired 11/19. Plan of Correction: I will make sure she goes to get a TB test.
Standard #: 22VAC40-185-260-A Description: Based on review of emergency records, there isn't documentation of an annual fire inspection. Evidence: There isn't documentation of a current fire inspection. Plan of Correction: I placed a call and emailed the fire inspector to get a copy of the inspection.
Standard #: 22VAC40-185-280-B Description: Based on observation, hazardous substances were not kept in a locked place making them inaccessible to children. Evidence: Hazardous substances were accessible to children in an unlocked cabinet in the preschool classroom Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-185-290-3 Description: Based on observations, not all electrical outlets did not have protective covers on them. Evidence: Electrical outlets were accessible to children and not covered with protective covers in the preschool and discovery classrooms. Plan of Correction: Corrected during inspection.
Standard #: 22VAC40-185-440-B Description: Based on observation, not all cots are identified for use by a specific child. Evidence: Cots in the discovery classroom are not labeled for use by a specific child. Plan of Correction: We will label each cot.
Standard #: 22VAC40-185-520-A Description: Based on observation, not all topical over-the-counter medications were labeled with child's name. Evidence: Diaper creams in the toddlers and discovery classrooms were not labeled with child's name. Plan of Correction: Corrected during inspection.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records, not all staff who have lived outside Virginia within the past 5 years have documentation of a Central Registry check for the state in which they had lived. Evidence: Staff #2 had lived out of state within the past 5 years and does not have documentation of an out of state background check for that state. Plan of Correction: I am sure I have received the out of state paperwork, but can't locate it at this time. I will continue to look for it or send out a new one.
