Licensing Inspector arrived at 10:00 and departed at 10:45 AM. At the time of the inspection, there were two children (infant & two-year-old) in care with two staff. Children were observed participating in free play. The sample size was two staff and two children's records.

Standard #: 22VAC40-185-70-A Description: Based on review of three staff records, it was determined that the facility did not ensure that all required elements were present in each staff member's records. Evidence: There was no record available for review for staff #3. Therefore, much of the required information was missing. Plan of Correction: Center director will ensure that there is complete record available for all staff members.
Standard #: 22VAC40-185-330-B Description: Based on an inspection of the playground, it was determined that the facility did not ensure that where playground equipment is provided there is resilient surfacing available (six inches) in the fall zone around the equipment. Evidence: 1. There was less than two inches of mulch immediately under the playground equipment. 2. The mulch did not extend to provide the required fall zone for the equipment. Plan of Correction: Center director will either purchase additional mulch or remove the playground equipment.
Standard #: 63.2(17)-1721.1-B-2 Description: Based on review of three staff records, it was determined that the facility did not ensure that all staff had completed fingerprint checks on file prior to beginning employment. Evidence: 1. Staff #3 did not have documentation of completed fingerprint checks on file. 2. Staff #1 stated that staff #3 had been working for "about two weeks". Plan of Correction: Center director will ensure that all staff have completed fingerprint checks on file prior to beginning employment.
