An unannounced renewal inspection was conducted on 12/19/18 from 11:30am until 2:30pm. At the time of the tour, there were 37 children in care with 8 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed eating lunch and resting quietly during nap time. Infant feeding and diapering, hand washing, restroom and table sanitizing procedures were also observed. Medication, transportation, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Three violations in the areas of administration, physical plant and special care provisions and emergencies are identified on the violation notice and were discussed with the center director during the exit interview.

Standard #: 22VAC40-185-130-B Description: Based on record review and interview, the center did not ensure that documentation of additional immunizations are obtained once every six months for children under the age of two years. Evidence: 1. The most recent immunization record for child 1, age 1 year, is dated 1/2/18. Child 1 has an enrollment date of 1/2/18 and an updated immunization record was due in July 2018. Plan of Correction: An updated immunization record for child 1 will be requested from the parent and placed in the child's record.
Standard #: 22VAC40-185-280-B Description: Based on observation and interview, the center did not ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. There was a bottle of Graffiti Remover located in an unlocked cabinet above the sink in the preschool classroom. The bottle was labeled "caution", "keep out of reach of children" and "avoid contact with eyes." 2. Staff 1 and the director confirmed that the container of Graffiti Remover was in an unlocked cabinet. Plan of Correction: The bottle of Graffiti Remover was placed in a locked cabinet during the inspection.
Standard #: 22VAC40-185-510-G Description: Based on a review of medication, the center did not ensure that written authorization from a parent or guardian is obtained in order to administer medication to a child that would normally be administered by a parent or guardian. Evidence: 1. There is an Epipen located in the center for child 2 with a written doctor's authorization dated 3/6/18 but no written parent authorization. 2. The director confirmed that there is no written authorization from the parent for the center to administer the medication to child 2. Plan of Correction: The parent had signed the expired medication authorization form but not the new form. The center will request that the parent sign the updated medication authorization form today at pickup.
