Comments:
An unannounced monitoring inspection was conducted today from approximately 10am-12:15pm. There were 83 children in the direct care and supervision of 17 staff. A sample of 8 children?s records and 5 staff records were reviewed. The children were observed playing with toys, singing, dancing, reading, eating snack, and engaged with teachers. Zero new staff members have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Description: Based on interview with staff, the center did not develop written procedures for injury prevention. Evidence: The center did not have written procedures for injury prevention. When licensing inspector asked for the plan Staff A stated that they didn't have one.

Description: Based on record review, children's records were incomplete.
