An unannounced monitoring inspection was conducted today, December 12, 2018 to determine the center's compliance with licensing standards. The inspection was initiated at 1:30pm and concluded at approximately 3:45pm. Upon arrival, the following ratios were observed: two infants/ one staff and four three-four year olds with two staff. Children were observed eating snack (goldfish crackers, bananas, and milk), listening to a short story read by staff, and preparing to go outdoors. All areas used by children were inspected to include the main hallways, children's restrooms, classrooms, and cafeteria. The center had the required postings displayed on the bulletin board upon entering the center. The center's first aid kit and emergency supplies were inspected with the required supplies found. Five children's records and two staff records were inspected with the required information present. The following was also inspected on this day: annual inspections, fire/shelter-in-place drills, parent agreements, and other pertinent information. Standard #: 22VAC40-185-160-A Description: Based on the inspection of staff records, the required tuberculosis (TB) test/screening was not present. Evidence: During the inspection of two staff records, it was discovered that staff #2 did not have a TB test or screening in the record. This documentation should have been submitted no later than 21 days after employment. Plan of Correction: Per director: I will have the staff to obtain a copy of the documentation from doctor and place it in the staff's record.
Standard #: 22VAC40-185-70-A Description: Based on the inspection of staff records, the required information was not listed in the record. Evidence: During the inspection of two staff records, it was discovered that both staff records did not have listed the date of employment and job position. Plan of Correction: Per director: I will update the staff records with the required information.
Standard #: 22VAC40-185-240-A Description: Based on the inspection of staff records, the required orientation documentation was not present. Evidence: During the inspection of two staff records, it was discovered that in both staff records, there was no documentation of orientation training in the record. Licensing standards require staff to receive orientation training by the end of their first day of assuming job responsibilities. Plan of Correction: Per director: I will provide the training and place the documentation in each staff's record.
