Comments:
The licensing inspector conducted an unannounced inspection on Tuesday, November 13, 2018 from 10:00am to 12:49pm in response to a self-reported incident that the Department received on October 31, 2018. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation did not support violations of the standards for the self-reported incident. Non-related violations were cited and are identified on the Violation Notice. Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Violations:
Standard #: 22VAC40-185-550-M
Description: Based on records reviewed on 11/13/2018, it was determined that the center did not maintain a written record of required information of children's injuries. Evidence: 1) One of one written injury reports reviewed did not include future actions to prevent recurrence of the injury. 2) One of one written injury reports reviewed did not include staff and parent signatures or 2 staff signatures. The injury report included only one staff signature (Child #1; injury report dated 10/31/2018).

Plan of Correction: Per the Center: During our last staff meeting all staff were informed and reminded on the correct way to write an injury report including future actions and signatures.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on a review of two staff records and interview on 11/13/2018, the center did not request, within 30 days of hire, a search of out-of-state founded complaints of child abuse for a staff member who has lived outside of Virginia within the last five years. Evidence: 1) During the inspection, the Sworn Statement for Staff #1 (DOH: 08/23/2018) indicated the staff member had lived outside of Virginia within the last five years. The records did not contain documentation of a requested founded complaint of child abuse from the state the staff member indicated she had lived in within the last five years. 2) During interview, a member of management confirmed a request for the out-of-state founded complaint of child abuse had not been requested for Staff #1.

Plan of Correction: Per the Center: I will request out of state child abuse forms for any new hire that has lived outside of Virginia in the last 5 years. While the inspector was here I contacted the out of state representative to obtain the correct form my employee to fill out which she did complete and it was mailed out.
