An unannounced monitoring inspection was conducted today, December 21, 2018 to determine compliance with licensing standards. The inspector was on site from approximately 10:30AM to 12:30PM. The following ratios were observed with a total of 43 children present during the inspection: Infants - 5 children with 2 staff Toddlers- 9 children with 2 staff Twos - 13 children with 2 staff Preschool - 13 children with 2 staff Pre-K - 15 children with 2 staff School age- 18 children with 2 staff During the inspection children were observed participating in holiday celebrations. Children played a game of unwrapping a cellophane ball to find candy, having a book exchange, watching holiday movies, singing Christmas carols and playing with toys. Bathroom breaks, hand washing, lunch and nap time were also observed. Areas of the center including classrooms, hallways, kitchen, and bathrooms were inspected. The playground was not inspected during today's inspection however documentation of additional resilient surfacing was reviewed. The facility was equipped with age appropriate toys and supplies for children in care. The center had the following information posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The following documentation was reviewed: injury reports, emergency preparedness plan, daily sheets, daily attendance, parent agreements, staff training information and liability insurance. The center's first aid kit and emergency supplies were inspected and found complete. Five employee records and six children's records were reviewed during today's inspection. The emergency preparedness plan was current. Emergency medications and authorizations were reviewed. Multiple staff have current MAT certification. The last documented evacuation drill was held on 12/19/2018. The two required shelter-in-place drills for 2018 were completed on on 04/19/2018 and 05/29/2018. A lock down drill was recorded on 05/29/2018. Last documented health inspection: 04/30/2018 Last documented fire inspection: 01/25/2018 Violations cited during the licensing period were checked for corrections. Standard #: 22VAC40-185-270-A Description: Based on observation, the facility did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: 1. The wooden top railing of a crib in the toddler room was broken. The split wood was taped but left a jagged edge that could snag the skin of a child. The staff reported that the broken crib was used by children in care. 2. The carpet in the school age classroom had tape around the edges to secure it to the floor. The tape was torn and created a loop that could trip a child in care. Plan of Correction: Per the director: We will repair (if possible) or remove the crib from the classroom. The crib will be removed from the classroom until repairs can be done. The rug in the school age room will be re-taped.
Standard #: 22VAC40-185-280-B Description: Based on observation, the facility did not ensure hazardous materials, such as cleaning materials, were kept in a locked place using a safe locking method that prevents access by children. Evidence: The cabinet under the sink in the toddler bathroom had a child safety lock however the lock was warped and did not secure the cabinet. The cabinet had multiple cleaning materials inside that were accessible to children in care. Plan of Correction: Per the director: The latch will be replaced in the toddler bathroom.
