An unannounced monitoring inspection was conducted with the director beginning at 9:20 AM and ending at 11:15 AM. There were five children, ages 4 and 5 years old, in the care of two staff members. The children were observed doing a holiday themed art project. Five children's records were reviewed. Four staff records were reviewed and in compliance. There were no medications on site to be reviewed. The outdoor recreational area was not inspected due to weather conditions. The emergency escape drill log was reviewed and in compliance. The first aid kit, flashlight, and weather-band radio were observed and in compliance. The most recent health inspection was dated 3/14/18. The most recent fire inspection was dated 11/5/18. Standard #: 22VAC40-185-130-A Description: Based on a review of documentation and staff interview, the center did not have documentation of a immunization for all children with prior to first day of attendance. Evidence: Child A, start date 9/6/16, did not have documentation of immunization in the record. Plan of Correction: The director stated that the documentation was in the child's record previously but could not be located during the inspection. The information was requested from the parents during the inspection.
Standard #: 22VAC40-185-140-A Description: Based on a review of documentation and staff interview, the center did not have documentation of a physical examination for all children with in one month after attendance. Evidence: Child A, start date 9/6/16, did not have documentation of physical examination in the record. Plan of Correction: The director stated that the documentation was in the child's record previously but could not be located during the inspection. The information was requested from the parents during the inspection.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all of equipment inside the center were maintained in a safe and operable condition. Evidence: There were two mats in the preschool classroom that had tears on the edges making them absorbent. Plan of Correction: The director stated that the mats would be removed from the center.
